TERMS AND CONDITIONS OF SALE
Cardholder agrees:
  1. This will be your first account with Dish Network, and that you have not previously had an account with Dish Network.
  2. You'll need to provide a valid major credit card, and social security number.
  3. The Free Dish Certificate sent to you from this purchase must be activated as your first and only account with DISH NETWORK within the intended household.
  4. You will call the 800 number on the Free Dish Certificate and activate it with America's Top 50 or America's Top 100 or America's Top 150 or Dish Latino within 10 days of receipt of shipment. You will maintain this programming for a continuous 12 months on all the receivers and will pay Dish Network the full amount of each invoice for the 12 months. You will not cut the programming off during this for any reason whatsoever.
  5. You will not activate the primary satellite system receiver  sent to you from this sale as slave or replacement receiver.
  6. All returned Free Dish Certificate must be received within 30 days of receipt, accompanied with copy of workorder. If the Free Dish Certificate is not received within the 30 days, all of the contractual conditions above apply in full. DO NOT TAKE OUT THE CERTIFICATE OUT OF THE PLASTIC or no refund will be given. Shipping costs are non refundable.
