*In addition to a $5.00 per month equipment rental fee per receiver, a $4.99 per month additional outlet programming access fee will be charged for each dual-tuner (DISH 322 or DISH Player-DVR 522) receiver. The $4.99 programming access fee will be waived monthly for each such receiver that is continuously connected to Customer’s phone line each month.

HERE'S HOW IT WORKS!!

DIGITAL HOME PLAN

    1You must be a new, first time residentail customer to DISH Network.
    2Pay a one time activation fee of $49.99. The $49.99 will be credited back to you on our first billing statement
    3You will receive your Digital Home Plan certificate in 7 to 10 business days.
    4When you receive the Digital Home Plan Certificate, you will then call to schedule your free professional installation. You'll need to provide a valid major credit card, social security number and agree to commit to 12 consecutive months of the Digital Home Plan. Participation in Digital Home Plan is subject to credit approval.
    5Your name and address associated with the credit card must match the name and address on the Digital Home Plan Certificate and the DISH Network account.
    6Select the Digital Home Plan of your choice, the number of receivers you would like and any additional programming choices (local broadcast channels, premiums, sports packages, etc.).
    7You will have to agree to keep the qualified programming for 12 months. If you downgrade from the qualified programming or deactivate your programming before the 12 months you will then be required to return all equipment back to DISH Network within 30 days after downgrade or deactivation in good operating condition. If the equipment is not returned in good operating condition within the 30 days, you will be charged by DISH Network for the damaged or missing equipment.
    8Florida residences will have to pay a 6% sales tax.
